Dr. Ariail retired on December 21, 2012.  The office is now permanently closed and no further services will be provided. Patient request for their medical records were available from Oct. 1, 2012 through March 31, 2013 at no charge for the first set of records.  After March 31, 2013 there will be a $20.00 fee for the processing and mailing of the records since the practice is no longer in business.  Please make checks (no credit cards please) payable to Ellen Horner and mail, along with your consent form, to the following address.
